1st Step

After you reach out to request an assessment, you will receive multiple documents to complete. This paperwork will include those of informed consent, confidentiality, and a good faith estimate as well as initial questionnaires.

2nd Step

After the review of the initial documents, a plan for scheduling your assessment appointments will be developed. Most people prefer to spread out the time as it is an extensive and time-consuming process.

3rd Step

You will complete additional questionnaires and attend appointments to complete the face-to-face (virtual) portion of the assessments.

4th Step

The questionnaires and face-to-face interview will be reviewed and a report will be written.

5th Step

You will return for an appointment to discuss the results of your assessment, any recommendations made, and to provide you an opportunity to ask questions about the report.
